HMO Products
Our Health Maintenance Organization (HMO) products combine excellent benefits with cost savings. You get all the health care and services you need by having a Primary Care Physician (PCP) coordinate your care. Your PCP will:

Provide all your routine medical care
Be available to direct or manage your emergencies
Admit you to the hospital when necessary
Refer you to a specialist

The PHPSM HMO products keep your out-of pocket costs to a minimum, while providing you with excellent health care. Our HMO products feature:

Low copayments
No claim forms to file
Access to our Health Management Programs, including the It's Your Life health management program
The only local health plan in the Jackson area
Network of over 1,000 Primary Care Physicians and Specialists
Access to 5 major hospitals, 35 outpatient facilities and 4 urgent care facilities
Prescriptions filled at hundreds of pharmacies
Each family member can choose their own PCP
You can change your PCP at any time

Plus Products
Our Plus products offer you more flexibility than HMO coverage by giving you two levels of benefits. You still choose a Primary Care Physician (PCP), and when you get health care through your PCP and other health providers in our network, your benefits will be paid at the lower HMO level.

Or you can choose to go to an out-of-network doctor, lab and/or hospital that is outside of the PHPSM network. If you choose to go outside of the PHPSM Network, you will pay a percentage of your health care costs and you will have a deductible to meet.

Either way, our Plus products will help you keep costs down while providing health benefits no matter where you live.
