Offering Plan K with 100% drug coverage
PHPSM is quoting Plan K as one of our high deductible health plan options. Plan K has a $1250/2500 medical deductible with 100% RX coverage after the deductible has been met. We have revised our HDHP benefit matrix to include Plan K. Delta Dental Products
PHPSM is offering four Delta Dental plans for new and renewal business. Benefit plan summaries, rates and employer applications can be found at PHPCares.com in the Agent portal.
New Phone System
In order to better route your phone calls, PHPSM has implemented a new automated phone system to accept calls when our receptionist is away from her desk. You can dial by name or extension, or can choose Option 3 to be transferred to the Sales Department Administrative Assistant. You can also call our direct-dial phone numbers listed on the Contact List below.
$$$ Broker Bonus Program $$$
We are pleased to inform you that the current broker bonus program will remain in effect through December 31, 2006 with minimal changes. To be eligible for the quarterly bonus, each broker must have at least four employer groups or 25+ subscribers that have a renewal/new effective date within the quarter. Bonuses will be paid out within 45 days of the end of each quarter.
FREE for New Members
PHPSM encourages new members (including agents who have PHPSM) to take our New Member Satisfaction Survey. This survey helps us evaluate our benefit and marketing materials. New members will be sent a Free PHPSM T-shirt for completing the survey. To complete the survey:
- Call 517-841-7477 to access the survey system.
- Press 7 to select the New Member Satisfaction Survey.
- Follow the voice prompts for completing the survey.
At the end of the survey, please include your name, address and t-shirt size to receive your FREE t-shirt. The t-shirt will be mailed to the address given.
Reminder: New Group Deadlines
To help ensure that the members are entered and ID cards are generated before a group's effective date, paperwork for new groups must be received by PHPSM a minimum of 15 days prior to the effective date. Required documentation includes:
- Group Policy Information Sheet
- Enrollment Forms
- Employment Verification: Prior Insurance Bill or MESC.

New Web-based Services Come to PHPSM!
PHPSM is preparing to implement our new online technology, HealthWeb, January 1, 2007. In order to make the transition, your current access to eService and eBill through United HealthCare will terminate effective October 31, 2006. We appreciate your understanding and patience during November and December. We think that you will find our new services worth the wait! HealthWeb will include the following capabilities:

- View Employer Invoices
- View Employer Receipts
- View Contract Breakdowns
- Online Member Enrollment
- View Member Eligibility
- View Member Benefit Information
- View Member PCP and COB information
- Provider and Facility Search
Phone disenrollment will also be terminated October 31, 2006. Formal communications will follow with instructions on how to submit information currently done on eServices or eBill as we get closer to the cut-off date. Balance Forward is Coming!
With the conversion of our systems on January 1, 2007, PHPSM will have balance forward capabilities as part of our new invoicing system. What does this mean to you? For new groups, this means that the premium check can (and must) be collected up front. Their first month's premium check will appear on their first bill, and will no longer show a payment due for the first month of coverage. Beginning January 1, 2007, please collect new groups first month's premium check with the Group Application. If you have any questions about our new balance forward system, feel free to call us at 517-782-7154.
Better Efficiency through Email
In order to improve efficiency, the PHPSM Sales Team has been sending proposals to you by email since June. This procedure has proven to be effective and efficient and allows us to better track when proposals are sent and to whom they are sent. We will continue sending you proposals in this manner. If you do not prefer this method and would like to receive a faxed or a hand-delivered proposal, please call Kristen at 517-782-7154.
Medicare Part D Update
Medicare has updated the content and language for the Notice of Creditable Coverage for subscribers who become eligible for Medicare Part D. The new notices are to be used after 5/15/06. Please advise groups that the notice has changed and the updated content/language must be used. The new notice can be found at www.cms.hhs.gov/creditablecoverage.
Notice of Creditable Coverage must be provided by the group to the subscriber: prior to becoming Medicare Part D eligible, before enrolling in the plan or if changes to the plan affect creditable drug coverage, prior to Medicare's "open enrollment" (11/15-12/31), and/or if a subscriber asks.
Passport Provider
Effective August 1, 2006, the commercial reciprocity arrangement, known as Passport, between Physicians Health Plan of Mid-Michigan (PHPMM), Physicians Health Plan of Southwest Michigan (PHPSWM) and Physicians Health Plan of South Michigan (PHPSM) will change. Beginning August 1, 2006, Physicians Health Plan of Southwest Michigan will no longer be a participant in the Passport Program.
If you have members who receive care from a Physicians Health Plan of Southwest Michigan physician or facility through the Passport Program, services after July 31, 2006, will no longer be eligible for reimbursement unless prior authorization is obtained. PHPSM has sent out contracts to physicians in Calhoun County and are currently awaiting responses. If a previously participating PHPSWM physician chooses not to sign a contract with PHPSM, that provider will be considered non-participating after July 31. There will be exceptions made for members that are in active treatment for such things as pregnancy, terminal illness, or chronic or acute medical conditions. Passport members may call 517-782-7154 to verify if their physician has signed the PHPSM contract and will continue to be a participating provider with PHPSM.
Please note that this reciprocity change does not impact the Passport Program between Physicians Health Plan of Mid-Michigan and Physicians Health Plan of South Michigan. The Passport Program will continue to remain in effect for PHPMM and PHPSM members.
