To be eligible for the quarterly bonus, each broker must have at least four employer groups or 25+ subscribers that have a renewal/new effective date within the quarter. PHPSM is pleased to announce that the National Committee for Quality Assurance (NCQA) has recently awarded PHPSM an accreditation status of Excellent. Our accreditation status - the highest level possible - was awarded after rigorous evaluation by NCQA of all aspects of our plan, including member satisfaction, quality improvement, preventive health services, and physician credentialing. The review included both on-site and off-site evaluations conducted by a team of physicians and managed care experts. A national oversight committee of physicians then analyzed the team's findings and assigned us an Excellent status based on our performance compared to NCQA standards. We are extremely proud of our achievement, not only in terms of what it means to us, but what it means to you - that you can feel confident in the quality of health care we provide to your clients.
New Dental Coverage offered through PHPSM
Effective October 1, PHPSM is pleased to offer dental coverage through Delta Dental Plan of Michigan. This will replace the current coverage being offered through United HealthCare Dental. Delta Dental offers:

New groups enrolled in a Delta Dental on or after October 1, 2005 will be offered guaranteed rates until January 2007 and a new annual maximum in October 2005, then again in January 2006. Social Security Number (SSN) Privacy Act
Effective January 1, 2006, Michigan law states that no more than four consecutive digits of a SSN:
- May be printed on Member Identification (ID) cards
- May be visible in any mailings sent by PHPSM
- May be used in the "subject" line in any e-mail sent outside of PHPSM, unless sent through an e-mail security system.
PHPSM will continue to collect and store Social Security Numbers. The SSN, however, will only be used:
- In the enrollment process
- To verify a members identity
- For claims processing services
Additional Network Available
In addition to the extensive local network that PHPSM currently offers, effective October 1, 2005 we will also offer the United HealthCare Options PPO Network to all members. The addition of this network means that HMO benefits apply at more physician's offices and healthcare facilities than before. It is also good news for college students and vacationers. A local PHPSM primary care physician should still be maintained, however, for coordination of healthcare needs. New cards with the Options PPO logo will be distributed to members over the next several months. A link to United HealthCare Options PPO providers can be found at our website www.phpcares.com. Medicare beneficiaries can now receive subsidized prescription drug coverage through a new Medicare Part D program. Employers who offer a prescription drug plan to Medicare eligible individuals are required by the Centers for Medicare and Medicaid Services to take action. Employers are encouraged to consult with legal and tax advisors who are familiar with their business needs if they have specific questions. PHPSM has a new automated survey tool to help us in assessing new member understanding of our written and verbal communications. Our new telephonic survey system, Telesage, records numeric and spoken responses to survey questions. The results are instantly stored in a standard Microsoft Access database, ready for analysis or automated reporting. It is immediately available for easy exporting and analysis. Information about the New Member Survey is distributed to new members in the welcome letter they receive with their ID cards. The letter explains the process by which they can telephonically complete the New Member Survey. The new member can also receive information about the survey from our web site at www.phpcares.com. New members will be sent a free PHPSM t-shirt for completing the survey.
