Member Packet Portal
Physicians Health Plan of South Michigan is pleased to announce that members now have 24-hour access to all of their coverage documents and member packet materials online through our "Member Packet Portal."

PHPSM members can view and print documents usually included in the member packet, regarding their Coverage. Members can also request hard copies through the mail by clicking on the link below. Materials in the Member Packet Portal include:

  • The PHPSM Certificate of Coverage
  • Member Handbook
  • Provider Directory
  • Other related documents
Through HealthWeb®, our new online tool, members can view their benefit summary, eligibility information, and deductible, co-pay, and accumulator information.
