Patient Safety Plan

In alignment with Foote Health System patient safety goals and National Committee for Quality Assurance (NCQA) 2004 Managed Care Organization (MCO) Accreditation standard Physicians Health Plan of South Michigan (PHPSM) collects information on practitioner and provider actions to improve patient safety and makes performance data publicly available for members and practitioners and providers.

One measure of patient safety is the percent of practitioners/providers that pass the ten (10) critical patient safety indicators of the credentialing facility site review (FSR) tool. PHPSM Medical Director, Dr. Dennis Means, and local network surgeon, Dr. Phil Frantzis, selected the final ten critical patient safety standards. These safety standards work to improve the quality of health care provided to members by giving them information they need to make informed health care choices.
