Throughout the year, PHPSM evaluates how well new members understand information regarding benefits and operating procedures. In an effort to monitor how well our written and verbal communications thoroughly and accurately represent our operating procedures, we are asking our new members to participate in a New Member Satisfaction Survey.

To complete the New Member Satisfaction Survey, please call 517-841-7477 and follow the voice prompts. The phone number is local to Jackson residents and will only take a few minutes to complete. At the end of your survey please tell us your name, address and t-shirt size, and we will send you a FREE Physicians Health Plan of South Michigan t-shirt as our thanks to you.

Your participation enables PHPSM to continue to improve our communications to new members. We would like to thank you in advance for your efforts in helping us provide you the best service possible.

  1. Call 517-841-7477 to access the survey system
  2. Press 7 to select the New Member Satisfaction Survey
  3. Follow the voice prompts for completing the survey. You may press the following buttons at any time:
    • Press # to skip to the next item
    • Press * to return to the previous item
    • Press 0 to repeat the current item
    • Press 9 to pause the survey
    • At the end of the survey, please include your name, address and t-shirt size (L, XL, 2XL) to receive your free t-shirt. The t-shirt will be mailed to the address given.
